Community Health Center is a Rural Health Clinic (RHC)
providing primary care services to Braxton County and surrounding areas.
Our medical staff and other employees are active participants in our
community.

Making appointments
Office visits are made by appointment only. Acute (sick or hurt) visits
are of course welcomed as time permits but we ask you to call for a
specific appointment time. Please understand occasionally emergencies or
complicated patients require more time and there may be a delay of your
scheduled appointment. We strive to make your visit as thorough and as
efficient as possible.

Billing:
We send statements on a monthly basis. Payment can be made by cash,
check or credit card. If you need to arrange a payment plan, our
Office Manager will be happy to assist you.
Insurance
We accept private insurance, Medicare, Medicaid, as well as other state
funded programs. We also have a sliding fee service available for those
without insurance who meet certain financial conditions. We do not accept
new patients for workers compensation cases. If you have insurance, we
will be happy to submit your claim for you. Patients are expected to pay
insurance or sliding fee co-pays at the time of the visit. If your
insurance plan requires you pay a deductible we ask you to pay that at the
time of your visit so we may submit this payment with your insurance
claim.
Patient Instructions
The Braxton Community Health Center is happy to have you under their care
and will make every effort to provide you with the best care possible.
The following items may help your first visit go smoother creating a more
efficient experience for you.
- Plan to arrive at least 15 minutes prior to your appointment time.
If you are a new patient coming for your first visit you should plan on
being here 30 minutes prior to your appointment to fill out the necessary
paper work.
- Bring your insurance information to every visit as well as your
medication or an updated and complete listing of all medication and how it
is prescribed. This information may be found on your pill bottle.
Please also include any "over the counter drugs" and "herbal"
medications on your list. If your child has a well-visit you will be expected to
have their immunization record with you at the time of the visit or you
may be asked to re-schedule.
- You will be asked to sign privacy statements that prohibit us from
sharing your personal information with anyone without written consent from
you. This includes your immediate family members.
- Children under 18 years of age must be accompanied by a parent or legal
guardian.
- To reduce the distraction and to eliminate confusion we ask that you not
bring any more than one person with you back to the examination rooms.
If you have multiple family members we kindly ask that you bring extra
supervision for the other members of your family so we may give each
individual our complete attention.
- While you are waiting to see the provider please be aware that there are
many providers that use our waiting area. If someone gets called
before you it probably just means they are seeing another provider that is
on a different schedule.
- We do accept walk-in patients as time permits.
These visits are worked into our regular schedule so patients with
appointments can be seen at their scheduled times. If you come in as
a walk-in and we can accommodate you, you will have to wait.
- You will be asked at the end of your visit to return a
paper to the front desk. We use this paper for billing purposes and
for follow-up appointments and it is important that you return it at the
time of your visit.
- After your visit should you have a question for the
provider or need a refill we ask that you give us 24 hours to return your
call and properly handle your request. We try not to interrupt
providers during the day for refills or questions so your appointment can
be timely. Also, when leaving messages it is important to be as
specific as possible. If all of your information is complete it will
help expedite your request.
